iLead Application 2019-2020
Thank you for your interest in the iLead Mentor Program. Please complete application in order to be considered for this opportunity. Carefully read all of the information included in this form and note the following important guidelines:

1. You must have an 80% average or above.
2. You must have good attendance.
3. You must have a clean discipline record.
4. You must be willing to commit to a drug and alcohol free lifestyle while being a mentor.
5. You must commit to one full school year in the program.
6. You must commit to twice a week mentoring sessions and agree to use that time for mentoring activities only.
7. You must agree to pay annual dues of $25 for program materials and supplies.
8. You and your parent/guardian must sign below.

iLead Peer Mentor Job Description

Mentor Role:
• Take the lead in supporting underclassmen through an ongoing relationship.
• Serve as a positive role model and friend
• Build the relationship by planning and participating in activities together.
• Strive for mutual respect.
• Build self-esteem and motivation.
• Help set goals and work toward accomplishing them.
Time Commitment:
• At least a one (1) year commitment.
• Communicate with mentee weekly.
• Attend an initial training session during the summer and additional group meetings as needed.
• Attend freshman orientation (date TBA)
• Mentoring sessions twice a week.
Desirable Qualities:
• Willing listener
• Encouraging and supportive
• Patient and flexible
• Tolerant and respectful of individual differences
• Willingness to pay close attention to students social, personal, and academic needs.
• Serve as a positive role model.
• Academically proficient.
• Sincere interest in helping freshmen and sophomores by committing to build a trusting relationship with each of their mentees.
• Knowledgeable about Mill Creek High School’s mission, vision, traditions, policies, procedures, programs, extracurricular opportunities and physical layout.
• Personal fulfillment through contribution to the success of a new student which helps to positively impact Mill Creek High School.
• Satisfaction in helping someone mature, progress, and achieve goals.
• Ongoing support and supervision to help your mentees succeed.
• A minimum of 40 hours of community service through teaching and leading your mentees.
• Leadership and service experience to list on college applications and/or resume.
