BPS Mandatory Learning Model Preference Form - OLSON ELEMENTARY SCHOOL

Bloomington Public Schools will return to school in a hybrid learning model, but we understand that some families may prefer a remote/online student learning environment. To finalize our plans for the reopening of schools this fall, we need every family to fill out this short survey to inform us of your plans.

IMPORTANT INSTRUCTIONS:

1. Please fill out this form ONLY for your children who attend Olson Elementary School.

2. To complete a form for children who attend another BPS school, visit https://bit.ly/BPS-preference.

3. PLEASE FILL OUT ONE FORM PER CHILD. There will be a link after you have completed the survey to submit another response if more than one child attends Olson Elementary School.
