Custom Piece Order Form
Please fill out this form to the best of your ability. Completion of this does NOT guarantee or secure you a custom order slot. Slots are only secured after I receive your deposit. There is a $20 non-refundable deposit that counts towards your total. I will contact you with questions regarding your piece and to make payment arrangements.

A final price will be given when the final details of your piece is discussed. I keep shipping costs as low as possible.  I do my best to complete your custom piece within 1-3weeks after you put in your order. If I am unable to accomplish this I will be in contact with you immediately.

It is important that you send my FB business page a message when you complete this form! I can't reach out to people directly unless they contact me first. A simple "custom order form submitted" is more than sufficient.
