CNAS MERCHANDISE REQUEST FORM
Please fill out the form below to request the CNAS merchandise for use at your event.

Rental requests from CNAS-affiliated departments and programs are considered on a first come, first serve basis. Requests must be submitted 2 weeks prior to event start date.

Note: Merchandise is distributed based on current availability. 

A member of the CNAS Communications team will contact you when your request has been approved. Your merchandise can be picked up from the CNAS Dean's Office (Geology 2258) 24 hours prior to your event. The CNAS Dean's Office is open Monday through Friday from 8:00 AM - 5:00 PM.
