Peabody Elementary Spirit Wear Order Form
The Peabody Elementary School PTA has the following Spirit Wear apparel available for purchase. Please know that certain sizes and colors may not be in stock for certain items. Finally, please be sure to review your final selections prior to submitting the order form below. Payment may be submitted to the school in the form of a check made payable to Peabody PTA or cash (exact change only). Please be sure to add your child's name to the memo line of your check. Once payment is received, your order will be processed and delivered to your student's teacher (unless special instructions are provided on the order form). He/She will place the order in your student's backpack to be brought home. PAYMENT MUST BE RECEIVED BEFORE YOU CAN RECEIVE YOUR ORDER.

The deadline to submit orders is Friday, September 1.
