Colorado Immunization Information System (CIIS) User and Site Update Form

Use this form to update an existing CIIS user account or to update information for a site previously enrolled in CIIS (clinic, hospital, pharmacy, LPHA, school, or child care).

If you need to enroll as a new site in CIIS, use the Colorado Public Health Reporting Portal (CoPHR).

If you need to update multiple users or site information for a medical group, school district, or child care group, email the CIIS Help Desk at cdphe.ciis@state.co.us.
