Fill out all fields completely, and submit this form for each date/request SEPERATELY! Only one event selected at a time. 

Remember: PLEASE MAKE AN EFFORT TO BE AT EACH EVENT! We understand if you need to miss certain events due to circumstances out of your control, but please communicate your absence beforehand.

This form must be filled out at a minimum ONE WEEK before a performance and 24 HOURS before a rehearsal. This allows ample time for Dr. Centeno/Program Managers/Drum Majors to approve your requested absence.
