This form is to be completed by IHF / PHECC Instructors (Or Scheme Coordinator) no more then 24 hours before they undertake a course under the National Ambulance Service Training Site. This is a risk assessment to ensure that the venue you are using is suitable for training.

Only those instructors who are delivering community based courses may operate through the NAS Training Site.

Clean, tidy and floor free from any obstructions which would pose risk

Access to audio visual facilities

Unobstructed access to facility

No slip risks present

No excessive noise levels that would impact training

Appropriate furniture for training (i.e. chairs, tables)

All training equipment has been cleaned before use.
