2025 Bridging & Youth Programme Registration Form (Closing Date: 15 Dec 2024)

IMPORTANT: Compliance with Guidelines for the Protection of Personal Data
(a) The collection, storage, retention, adaptation, modification, reading, retrieval, use, transmission, blocking, erasure or destruction (“Processing”) of the personal data provided by me in this form (“Personal Data”); The church entity processing my or my child’s Personal Data for the purpose of my registration for a course by the church entity.
(b) The personal data provided will be retained for archival purposes for a period of seven (7) years, thereafter all personal data will be deleted/destroyed appropriately according to PDPA guidelines.
(c) The church entity transferring my or my child’s Personal Data to other church entities within the Catholic Archdiocese of Singapore.
(d) The Office For Catechesis (OFC) taking photos, videos, or audio recordings which may contain my or my child’s image/audio during OFC or OFC related events and may be used for archival purposes, on Office for Catechesis (OFC) website, publications, OFC's Training and Courses and for publicity purposes.
