I understand that the enrollment contract with the school financially obligates me to pay the full amount of tuition and fees for my child to attend regardless of withdrawal for any reason. *
I understand that changes made to the class schedule are $50.00 per class change (part time students only) after the schedule is agreed upon with FACTS. *
Volunteer hours (full time day school) are important for the health of the school and community.  I agree to the following:

- 5 hrs per quarter (20 total) for family volunteerism.
-5 hrs per quarter ( 20 total) for 6th-8th grade students. 
-10 hrs per quarter (40 total) for 9th-12th grade students.  High school students hours may count toward the family volunteerism, but family hours do not count toward the student.  

The volunteer hours will show up on report cards on the semester.

Media Consent I give permission to include, but is not limed to, use the following: pictures sent via email from a class trip, classroom function, activity, dance, media class editing team projects (yearbook), for the yearbook club to create a spot in the yearbook, and possible advertisements within the community.  This permission is a family consent for parents/guardians, students, and siblings who attend any activity.   *

-Snacks and lunch items that teachers or students bring.

-Food brought in by professional organizations to share with the school.  These items are usually cereal, snack bars, and other prepackaged items.

Note-  I understand that some food is made in the kitchens of parents, grandparents, church staff, or teachers.
