Foreword: Filling and submitting this form is the first step of your application towards membership of CSI Church Sydney. The decision on membership will be taken by the Pastorate Committee after which you will be provided with the printed copy of this form with all the data entered. This has to be signed and submitted for Church records and the final step will be welcoming and introducing you as a member during one of our Church services.
