Waiver of Liability by registering for and/or participating in the NMHS Junior Jag Youth Cheer Camp, I, the parent and/or legal guardian of the undersigned, acknowledge and agree to the following terms and conditions: I understand that participation in cheerleading activities involves certain inherent risks, including but not limited to physical injuries, sprains, fractures, or other accidents that may occur during training, performances, or any other related activities. I voluntarily assume all risks associated with my child’s participation in the NMHS Junior Jag Youth Cheer Camp. Release of Liability: I, for myself, my heirs, executors, and assigns, hereby release and hold harmless NMHS Junior Jag Youth Cheer Camp, its coaches, staff, volunteers, and any affiliated organizations from any and all liability, claims, demands, or causes of action that may arise from his/her participation in the camp, whether caused by negligence or otherwise. Medical Emergency Authorization: In the event of an emergency, I consent to the administration of necessary medical treatment and transportation to a medical facility, should the need arise. I understand that I am responsible for any medical expenses incurred. Code of Conduct: I agree to abide by all camp rules and regulations and follow the instructions provided by camp staff and coaches. I understand that failure to comply may result in my dismissal from the camp without refund. Photography and Media Release: I grant permission for NMHS Junior Jag Youth Cheer Camp to use photographs or videos taken during the camp for promotional purposes, including but not limited to websites, social media, brochures, and advertising, without compensation. Assumption of Responsibility for Personal Property: I acknowledge that I am solely responsible for my personal belongings and that NMHS Junior Jag Youth Cheer Camp is not liable for any loss, theft, or damage to personal property during the camp. *
