All nominations for the Southern Nash High School Athletics Hall of Fame will be submitted through this form, via online or paper versions.  Paper forms can be requested from the Southern Nash High School main office.  For consistency in nominations, no materials outside of this form will be considered.  All information provided on this form is subject to verification by the Hall of Fame Committee.  Nominees do not need to be nominated more than once; if multiple nominations are submitted, information will be consolidated into a single nomination.  Posthumous nominations are welcome.
