This form is for the confidential referral to the school social worker, Candice Clarke. The school social worker assists families in identifying and addressing barriers to school attendance, connects families to school and community resources, and refers qualifying families to McKinney-Vento (a federal program for families without stable housing).

Confidentiality- Information shared with the school social worker is confidential. The student’s right to privacy is guarded as much as permitted by law, ethics, and school policy. The school social worker is obligated to break confidentiality when there is potential harm to the student or others, concern of neglect/abuse, or a court of law that requires testimony or student records.
At times, the social social worker and school-based staff (teacher, counselor, principal, etc.) need to exchange information about students. Such communication takes place on a need-to-know basis.
