The Course Materials will be made accessible online through a shared Google Drive folder (Google accounts required). Access to this folder is restricted to those who have completed online registration and payment only. Printed copies of session handouts will no longer be provided, thus, participants should bring their laptop/Mac/any device for access to the materials.
To ensure the quality of instruction, we limit the number of participants for each course. Admission is on a first-come-first-serve basis, especially for those that quickly run out of slots, such as the ICD-10 training course. If participants are keen on attending the course, they are advised to register at this form as soon as possible. For further information, send email to firstname.lastname@example.org, or contact us at (02) 310 4203, telefax (02) 523 2997, or through facebook.com/DHPAShortCourses/.
A non-refundable reservation fee of ₱1,000 may be required for confirmed participants who cannot give the full payment yet. This is then deducted from the full payment, and the balance may be paid on the first day of the course. No slots will be reserved if no payment is made.
Registration and payment should be completed two weeks before the start of the course. The department reserves the right to cancel the course or to refuse walk-ins when slots are no longer available. If this happens, confirmed participants will be notified by email and all payments will be refunded. For confirmed participants who cannot attend the course, they should advise the office two weeks before the start of the course, otherwise, payment is non-refundable.
