I am aware that there are risks and hazards involved in practicing a martial art using wooden and metal swords.
I understand that it is my responsibility to consult with a physician prior to and regarding my participation.
I represent and warrant that I am physically fit and I have no medical condition(s) that would prevent my full participation and accept that it is at the discretion of the instructor to suspend my practice should they have any concerns about my physical and/or mental health.
I acknowledge that I agree to assume full responsibility for any risks, injuries or damages that may result from my use of my equipment.
Instructors will also have the right to inspect and prohibit the use of any equipment at their sole discretion.
In further consideration, I knowingly, voluntarily and expressly waive any claim I may have against Etobicoke Olympium Kendo Iaido Club, City of Toronto, Etobicoke Olympium, their owners, directors, officers, employees, instructors, members, volunteers, invitees and representatives, for any injury or damages that I may sustain as a result of my participation.
Permission for Photographs & Videos
Photographs and videos will be taken from time-to-time that may be posted on the Etobicoke Kendo Iaido website and through social media. If you do not wish to have your photo or video posted online, please let us know at registration time of the event.
