New Excused Absence Form

Please fill out the form INDIVIDUALLY FOR EACH DAY THAT YOU WILL MISS below to have an absence excused. If you will be out THREE DAYS OR MORE, please scroll to the bottom and type in the date range.

In most cases, absences are excused if a parent/student gives warning 24 hours or more in advance of class or within 24 hours after class. If not, we will contact you. Doctor's notes are not needed.

Students with more than three unexcused absences per semester:
1. May lose any merit scholarships or reduced tuition support
2. May not be eligible for financial assistance or merit based support the following year
3. May not perform in the year-end dance recital or other performance opportunities
