Reason/s you are applying for exceptional and unavoidable absence from school *
Has your child been granted exceptional leave previously? *
I apply for my child to be granted authorised leave of absence from school for the exceptional and unavoidable reasons stated. I understand that if this is not agreed, then any leave subsequently taken will be treated as UNAUTHORISED ABSENCE and could result in the Local Authority issuing both parents/carers with a Penalty Notice Fine of £80 each, rising to £160 if not paid within the stated time. I understand that there will be an automatic £160 fine if I take my child out of school for unauthorised leave for a second time.

*Please note that after 2 incidents the Local Authority may decide to prosecute which could mean a fine of up to £2,500
By clicking submit you understand and agree to what has been stated above. Submission of this form does not mean the exceptional leave has been granted. We will write to you to let you know the decision.
