This form is meant to collect any concerns that you have observed regarding protocols not being followed. LCUSD has committed to protocols listed in the Los Angeles County Department of Health "Reopening Protocols for K-12 Schools." You may use this form as often as needed. LCTA will submit any concerns to the district at least once per week. PLEASE NOTE all concerns will be delivered to the district by LCTA and members will remain anonymous. We are only asking for emails for the purpose of LCTA being able to follow up with members.
