Canine Addison's Survey
This very brief survey will be used as a tool to collect information about Addison's medications to help owners & vets learn more about the doses of medications that keep our dogs feeling their best. CARE, along with members of the Canine Addison's worldwide community have joined together to publish this survey to help Addison's dogs throughout the world. If you would like to provide your email address in the survey, we will periodically send updates about the survey results.
If a question doesn't apply to your dog, skip that question & move on to the next one. If you have more than one dog with Addison's, you can fill out more than one survey. If your dog has since passed on, you can still fill out the survey & let us know what medications your dog had been taking.

If on Percorten, what is the dose in ml & how often is it given? Is it given IM (intramuscular) or SQ (subcutaneous)?
If on Zycortal, what is the dose in ml & how often is it given?
If on Percorten or Zycortal, are you still reducing the dose?

If on Florinef, Fludrocortisone, or Astonin-H, what is the TOTAL daily dose in mg? For example, if you give 0.4 mg twice per day, the total daily dose is 0.8 mg. (There has been some confusion: We are NOT asking for the total number of tablets given per day.) If you're still adjusting the dose, please let us know.
If on Florinef, Fludrocortisone, or Astonin-H, do you split the dose & give it twice per day as is often recommended?
If on Prednisone or Prednisolone tablets, what is the dose in mg & how often is it given? If you're still reducing the dose, please let us know. (For dogs on Florinef/Fludro, who don't need daily Pred, it's okay to report the amount of Pred & write "as needed" -- or write "daily" if that's what you do.)
If on liquid Prednisolone, what is the dose in mg or ml, how many mg/ml, & how often is it given?

How is your dog doing overall, as it relates to his or her Addison's treatment? (If your dog has since passed on, please rate how your dog had been doing with his or her Addison's treatment.)

Optional - Has your dog switched from one medication to another? If so, please let us know the previous medication, dose, & frequency & the reason for switching. Or, if your dog was started on higher doses of the current drugs, let us know what the original doses were & what caused you to reduce.
