Congratulations! You are ready to take the next step in your journey towards faith and freedom. Thank you for choosing PAX Ministries for guidance in the establishment of your unincorporated entity. We are thankful for your trust and your mission to help others.

By submitting this form, you are entering into a private contract with PAX Ministries and agree to the following terms. Terms are as follows:

The Ministry will gift to you (Member), either in person or in PDF format via email, the founding documents for your new Association. This gift package will include:

  • Charter/Bylaws,
  • Articles of Association,
  • Membership Application,
  • Intake/Introduction, and
  • Certification of Execution (Ministries only)
We will also share with you the Practice of PMAs eBook and online course (upon donation receipt) as well as additional resources to aid your journey. You will receive one (1) rough draft of your documents within 8 calendar days of donation to which you may propose edits via email, phone, or video conference. This is the sole opportunity to offer your revisions or request additional needs. The revisions will be made, and you will receive the final document, either in person or in PDF form, via encrypted email, within 4 additional days. Should you need to revise your Association’s founding documents in the future, this is accomplished via meeting minutes, which you will document for the Association’s internal records.

Member agrees to indemnify PAX Ministries for all costs, expenses, judgments, fines, and penalties incurred resulting from the negligent conduct of the other party, or their employees, managers, principals, agents, or sub-agents.
