IWA Community Care Team Referral Form
Adolescents experience great physical, emotional, and social adjustments throughout out their high school years. Care Team was originally created to address the needs of students from an interdepartmental process (Administration, Counselors, Learning Consultant, Teachers, Campus Ministry). Allowing faculty and staff at IWA to collaborate and aid students. As a means of being an additional support and safe learning environment this year the IWA Counseling Department has created the IWA Community Care Team Referral Form. This form can be used by caregivers and students to express concerns for themselves, students, or others. Concerns can range from academic, mental health, behavior, social/emotional, safety, etc. IWA hopes with awareness and communication we can aid in helping students achieve their potential by working through challenging issues. All IWA Community members may suggest students in need of Care Team assistance. The team gathers information about the student’s specific situation, designs an individualized plan to help the student, and continues to monitor her until the problems have been resolved.

The Counseling Department is typically available from 7:45 a.m. until 3:30 p.m. on school business days.
