Tilden Attendance Notification 2025-2026
Dear Parents and Guardians,

Please complete the form to communicate a full day absence. A doctor's note is required if your student is absent for 3 or more days due to illness and/or injury. If you have any other attendance questions, or your child is leaving early, please see our Attendance Policy.

A phone call and/or voicemail will not excuse an absence, late arrival, or early dismissal. Documentation is required.

We also accept handwritten paper notes in the format of: Attendance Note form 25-26. The handwritten paper note must include:
  • Student's full name, ID number, and grade
  • Date(s) and reason(s) for absence(s)
  • Parent signature and best contact number
