I/We and any member of my family have never been subject to a formal complaint or legal action for cruelty or neglect of any animals and have never owned animal(s) which have been confiscated by any agency for violations of any laws, regulations or animal adoption agreements. *

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any animal rescue animal fostered by me. *
I/We give my full agreement and assurance that all dogs will be fostered as domesticated house pets and will all sleep inside my/our house at night. *
I accept full responsibility for the animal(s) actions at all times, and release Tania Dicks Maurice (Paws - Restart)  from any liabilities or damages that may be incurred because of fostering. *
I/We agree that Tania Dicks Maurice (Paws - Restart) will not be held liable or accountable in any way for any damages incurred by any dog in my care and that I assume all responsibility to repair or replace any damaged items, fixtures or fittings and have adequate contents insurance cover to meet any damage that may occur. *
I agree to have Paws - Restart complete reference call checks and conduct a home visit inspection to be able to approve my foster application. *
I agree that if I'm unable to foster the foster animal(s) anymore that I will return the dog(s) to Paws - Restart and try to give  a 2 week period to try and find another suitable foster for the animal(s). *
I/We understand that if at any time Paws - Restart has any concerns about the care, health, safety or welfare of any or all of the puppies and/or dogs in my/our care, Paws - Restart  has the right to uplift and take back any or all fostered animals. *
Where required I/We give consent to Paws - Restart to take urgent action without notice if it is determined that the any animals in foster care are at risk. Failure to hand over fostered animals may result in Police action. *
Do you agree to weekly communication with Paws - Restart by using the private Facebook group and using this group for any questions and concerns you may have? *
Please confirm that all training methods suggested will be followed or discussed before implementing change. *
I/we agree to giving at least 2 weeks notice when supplies need replenishing? *
I/we agree to making contact straight away if any medical or behavior issues arise? *
