MILLBRAE AYSO YOUTH SCHOLARSHIP PROGRAM APPLICATION FORM
Millbrae AYSO Youth Scholarship Program is designed to provide financial assistance to the participants of Millbrae AYSO in need to enable them to participate in AYSO programs.

PROCEDURES and GUIDELINES

Based on the availability of funds, Millbrae AYSO will attempt to provide youth scholarship to those participants who meet the eligibility requirements. Millbrae AYSO reserves the right to approve or deny an applicant’s request. The following guidelines have been developed to help make our programs safe and enjoyable for all participants.
  1. The Youth Scholarship application is applicable only for the Fall program.  
  2. The Youth Scholarship application must be completely filled out and returned with the registration form in order to be considered. The application will be reviewed and processed by Millbrae AYSO.  
  3. Upon submission of a scholarship request, families will be notified within ten (10) business days after the last registration date of their scholarship status.
  4. Youth Scholarship is based on need and availability of funds and will be awarded on a first come-first serve basis. This scholarship covers Region expenses and families will need to pay the National Fee 25.00 and Processing Fee 3.50 (These fees are non-refundable as the Region does not process the fees).
  5. All submitted information is confidential and is not a matter of public record.
  6. All information on Youth Scholarship Program application must be true and accurate.
  7. Youth Scholarship awarded on the basis of false information supplied by the applicant will be revoked.
  8. Youth Scholarship is limited to one per person per season. Applications must be completed each season. Granting of approval does not ensure continued approval for succeeding seasons.
  9. Families who receive a youth scholarship must re-apply with each season.
  10. All registration policies and procedures apply to scholarship applicants.
  11. There are no refunds for scholarship money that has been awarded.
  12. Each Youth Scholarship award recipient is recommended to volunteer for Millbrae AYSO.  Specific assignment is at the discretion of Millbrae AYSO.
  13. The Millbrae AYSO Youth Scholarship Program is subject to change.

Youth Scholarship Application Questions

In order to be considered for the Millbrae AYSO Scholarship program, this form, plus the Millbrae AYSO registration form must be completely filled out and submitted with the required documents. Following the verification of the information provided on this form, applicants will be notified within 10 business days or less after the last registration date of the status of your request. The Millbrae AYSO considers all requests and takes into consideration current circumstances. If awarded a Youth Scholarship, we ask recipients of scholarships to do their best to volunteer a minimum three (3) hours for Millbrae AYSO before the requested Fall season is over, or take on a Volunteer Position. This is not to create hardship for individuals but to allow the individuals to assist the Region as the Region takes on the cost for the individuals, and any Volunteer help greatly offsets the extra costs and Volunteer time to mitigate the costs to the Region.
