Official Transcript Request 
Please allow 48-72 hours for the transcript request to be finalized. Please do not Duplicate your request. 

Please read carefully: Official transcripts must be sent directly from SHS to the college, university, scholarship, etc. If you are currently enrolled at SHS, you can access an unofficial copy of your transcript from your Skyward Portfolio in "attachments".

* Current students: we are unable to send transcripts through Parchment; you must provide an official transcript email for the school or create an account in Slate.org.  
* Former students may request a copy be sent to them, but it will not be an official copy. By completing this form, you are assuring that you are the student or the legal guardian (if under 18). There is not a fee for transcripts at this time.

MTSU does not accept transcripts through email, you must create a Slate account for current students only. All past students MTSU transcripts must be sent through the mail so please allow 2 weeks for MTSU to process mailed transcripts.
