This form is intended to be filled out once, per child, per trimester. We will have seasonal sign-up periods for our students based on the new and seasonal offerings we provide through out the school year. Please use this form to select activities your child would like to enroll in for the Spring Season (March 25 - June 14). **Please note: Volleyball will start on April 1st.
