PLEASE READ THE INFORMATION BELOW CAREFULLY. YOU ARE REQUIRED TO READ AND UNDERSTAND THE INFORMATION CONTAINED IN THIS DOCUMENT. BY SIGNING THE REGISTRATION FORM YOU ARE INDICATING THAT YOU HAVE READ AND FULLY UNDERSTAND THIS DOCUMENTATION. FAILURE TO SIGN THE REQUIRED FORM WILL RESULT IN YOUR IMMEDIATE DISMISSAL FROM THE PROGRAM.
The head coach from your team MUST be called if you are not going to make to a practice or a game. FAILURE TO NOTIFY THE HEAD COACH MAY NULLIFY YOUR MANDATORY PLAYING/CHEER TIME. Coaches may set minimum attendance penalties for practices for participation in games and/or competitions.
Respectable behavior is expected at all times from both the players/cheerleaders and the parents during practices and games. PARENTS ARE NOT PERMITTED ON THE GAME OR PRACTICE FIELDS WHILE PRACTICE OR GAMES ARE TAKING PLACE. THE PIRATE COACHING STAFF HAVE THE RIGHT TO REMOVE ANY PERSON FROM THE PRACTICE OR GAME FOR DISORDERLY CONDUCT.
The equipment that is handed out to your child is the property of the Palisades Youth Football Association and is expensive to replace. While it is in your possession, it is your responsibility to make sure that it is washed properly and doesn’t get lost or stolen. You will be required to make restitution to our organization for any lost or stolen equipment at the current market value. (This does not include equipment damaged during normal usage at practice or games) Washing instructions are available on www.PPYF.com.
Your child will not be permitted to participate in any practice if the correct paper work has not been returned prior to the start of the season. NO CHILD WILL BE PERMITTED TO ENGAGE IN PRACTICE WITHOUT THE PHYSICAL FORM, NO EXCEPTIONS.
AT THE CONCLUSION OF THE SEASON YOU WILL BE ADVISED OF THE DATE AND TIME FOR EQUIPMENT RETURNS. IF YOUR CHILD/CHILDREN’S EQUIPMENT IS NOT RETURNED ON THIS DATE YOU WILL BE BILLED FOR THE CURRENT MARKET VALUE OF THE MISSING EQUIPMENT AND BANQUET TROPHY(S) WILL BE HELD UNTIL THE AFOREMENTIONED IS SATISFIED.
THERE ARE NO REFUNDS OF REGISTRATION FEES, UNDER ANY CIRCUMSTANCES.
ANY DISCREPANCIES OR ISSUES, MONETARY OR OTHERWISE, REGARDING THE CURRENT SEASON MUST BE ADDRESSED AT AN ORGANIZATIONAL MEETING PRIOR TO THE END OF NOVEMBER OF THE CURRENT SEASON. THERE WILL BE NO EXCEPTIONS TO THIS RULE.
PARENT/PLAYER RULES AND CODE OF CONDUCT
I hereby understand and acknowledge that as a parent/guardian of the Palisades Youth Football & Cheer Association (PPYF) participant it is my responsibility to comply with all rules and regulations stipulated, adopted or recognized by PPYF or any of its member organizations, including but not limited to the Parent/Player Rules and Code of Conduct. Any non-compliance with any and all rules and regulations may be cause for discipline and/or dismissal of my child/the participant, myself, and/or any spectators or other persons affiliated with the undersigned and the above named participant. I understand and hereby waive my rights to a hearing for any suspension by my Association and/or League of (2) weeks or less.
By my providing my initials below, I hereby stipulate that I have read, fully understand and voluntarily agree to all of the above.
I agree that PPYF & CYFL may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
I hereby understand that my child’s information may be shared between CYFL administrators in a password protected area of CYFL website at www.leaguelineup.com/CYFL
Code of Conduct/Photo ReleaseAs a Parent, Guardian, Coach, Assistant Coach, Team Mom or Dad:-I hereby pledge to provide positive support, care and encouragement for my child and other children participating in youth sports by following this Code-of-Conduct.-I will encourage good sportsmanship by demonstrating positive support for all players, coaches and officials at every game, practice or other youth sport events.-I will place the emotional and physical well-being of my child and other children ahead of any personal desire to win.-I will insist that all children play in a safe and healthy environment.-I will provide support for coaches and officials working with children to provide a positive, enjoyable experience for all.-I will demand a drug, alcohol and tobacco-free sports environment for all children and agree to assist by refraining from their use at all youth sports events.-I will remember that a game is for children and not for adults.-I will do my very best to make youth sports fun for each child.-I will ask my child to treat other players, coaches, fans and officials with respect regardless of race, sex, creed or ability.-I will promise to help my child enjoy the youth sports experience within my personal constraints by assisting with coaching, being a respectful fan, providing transportation or whatever I am capable of doing.-I will discuss this Code-of-Conduct with my child/children.-I will bring issues of concern to the CYFL Board for discussion.-I understand and accept the Code-of-Conduct as written above. I also understand that failure to comply with this Code-of-Conduct will result in disciplinary action as determined by the CYFL after a review of the violation. I agree to abide by the CYFL findings and realize that this includes but is not limited to, removal of my child from the sport.Photo Release
I grant to the CYFL, its representatives, volunteers, directors, coaches the right to take photographs of me and my property in connection with any sports activity conducted by the CYFL. I authorize the CYFL, its assigns and transferees to copyright, use and publish the same in print and/or electronically.
