Hillsborough County Council PTA/PTSA Awards Master Submission Form
The Master Submission Form must be completed on or before 11:59 pm, March 8, 2019. No applications will be accepted after the deadline. All narratives MUST be entered into the appropriate field. No attached narratives will be considered. Attachment fields are for supplemental materials only. All fields for each award are required when applicable.
Incomplete applications will not be considered. After completing the form, you will receive an email copy of your responses. You may edit your submission anytime up to the deadline.

You should have all of your materials ready before beginning this application. You can edit your responses after you have submitted, but there is no 'Save & Return Later' functionality. We recommend having everything in a Word document for easy cutting and pasting.

PTA/PTSA's, their members, and other Individuals who have made significant contributions to the well-being of the Hillsborough County School District Community may be considered for Awards. Local units must be in good standing with County Council and Florida PTA to be eligible to win.

- To advocate for laws that further the education, physical and mental health, welfare, and safety of children and youth;

- To engage the public in united efforts to secure the physical, mental, emotional, spiritual, and social well-being of all children and youth; and
- To advocate for fiscal responsibility regarding public tax dollars in public education funding.

Outstanding High School Senior/Student Led Program
Purpose: This award is to recognize an outstanding senior for a student led program, project, or event that they created. This can be a "legacy" program that they created as an underclassman and has carried forward. It is the intent of HCC PTA/PTSA to give this award to a senior who may not necessarily be top running for scholarships, but has well rounded character, supported your PTSA through a program, and demonstrates maturity consistent with being a good citizen.

Criteria: The Senior's participation must be based on this school year's activities, unless it is a "legacy program" that they created as an underclassman and is still being used. Only one nomination per school is allowed. Your PTSA chooses the senior and each nominated senior will be recognized and receive an award.

Purpose: This award is to recognize an outstanding PTA member in a local unit ( not President or Principal), who has been instrumental in developing programs and/or demonstrated service/leadership in promoting programs that support the Purpose of PTA. The nominee must have promoted at least TWO PTA goals. Tell us about their involvement, leadership, ideas, planning and extraordinary efforts in PTA.

All forms of communication will be considered, and looked at as a whole. Be sure to submit any communication channels you utilize to allow the judges to get a complete picture. All listed items are not required, but you are encouraged to submit as many as apply to your local unit.
