2026-2027 Registration Form Season #15

Welcome to Season #15!
We are so excited to welcome you back—or if you’re new, welcome to the Elite Dance Academy family! 💜

Each season, a new registration form and updated credit card authorization form are required. We understand this may feel repetitive, but it ensures we have the most accurate and up-to-date information for your dancer and allows us to communicate effectively with you throughout the season.

All families are required to be enrolled in autopay, with the option to have payments processed on either the 1st or the 5th of each month. A $25 NSF fee will be applied to any payment that is declined.

Please complete all required fields to register your dancer(s). Once your form is submitted, our team will finalize your registration using the information provided. This includes processing payment for:

  • Annual Standard or Premiere Membership Fee
  • First full month of tuition

Please note: The first full month of tuition is required at registration. Any prorated tuition will be applied to the second month.

Registration is not complete until payment has been successfully processed. Dancers cannot be enrolled in classes until registration is finalized.

Annual Pay Option:
Families who choose to pay tuition in full for the year will receive 10% off monthly tuition and no Annual Standard Membership Fee (VIP Membership fees are not included in this offer).

If your card is declined at the time of registration, you will be contacted and given 24 hours to submit payment. If payment is not received within that timeframe, a new registration form will be required.

Important Policies:

  • Tuition and membership fees are due at the time of registration
  • The card provided will be used for autopay for the entire season
  • All registration fees are non-refundable and non-transferable once submitted

We can’t wait for an amazing Season #15 with you! ✨

Tuition:
Tuition is based on 12 months divided into 12 equal monthly installments. Tuition remains the same each month regardless of seasonal breaks or whether a month has more or fewer than four weeks.

No refunds or credits will be issued, including for missed classes.

Payment of Fees:

All families are required to enroll in automatic payment for dance tuition to ensure convenience and timely processing. Families may choose one of two payment processing dates: the 1st or the 5th of each month, allowing you to select what best fits your financial schedule.

Payments will be processed on your selected date. Any declined payment will incur a $25 NSF processing fee. It is the responsibility of each family to ensure that card information is accurate and up to date, the card is unlocked, and sufficient funds are available on the scheduled payment date.

Effective June 1, 2026, qualifying credit card transactions will incur a 3% credit card processing fee. This fee applies to qualifying Visa and Mastercard credit card transactions.

Accounts with unpaid balances after the 7th of the month will result in the dancer being unable to attend classes until the account is brought current.

Late Fee:
A $25 late fee per student will be applied to any unpaid tuition after the 7th of each month.

Tuition is non-refundable and cannot be transferred or credited to another student’s account.

Forms of Payment Accepted:
EDA accepts cash, debit cards, and Visa or Mastercard credit cards. No hundred-dollar bills or checks accepted.

Cash payments are accepted; however, a valid card must remain on file at all times. Families paying with cash must submit payment at least 24 hours prior to their selected automatic payment processing date to prevent the card on file from being charged automatically.

Please plan ahead—if the 1st or 5th falls on a weekend or holiday, cards on file will still be processed on that day

Acknowledge by Initialing Below:

Withdrawals and Refunds:
A written 30-day notice is required to withdraw a student from any class(es). Drop notices are only accepted through the official EDA Drop Notice form, which can be found on our website under the “Forms” tab.

Verbal requests, emails, phone calls, or notifications given to instructors will not be accepted. It is the responsibility of the parent/guardian to complete and submit the official drop form.

Tuition will continue to be charged until the official drop notice has been completed and submitted. Non-attendance does not constitute a withdrawal—please do not assume your dancer has been dropped due to missed classes.

Families are responsible for all tuition during the 30-day notice period, regardless of attendance.

No refunds or credits will be issued—no exceptions

For safety reasons, no jewelry may be worn during class.

Hair must be neatly secured in a bun for all classes.

Dancers who are consistently out of dress code may be asked to sit out of class. Parents will be notified, and continued failure to follow the dress code may result in termination of lessons.

Important Attendance & Make-Up Policy Reminder

Make-up classes are permitted during July and August to accommodate planned summer vacations. All absences during this time must be made up within 30 days.

Beginning October 1st, make-up classes are no longer allowed—no exceptions.

Please note: We do not offer refunds, tuition reductions, or prorated rates for missed classes. Each class is structured specifically for its enrolled students, and make-ups can disrupt the learning environment. Similar to sports, missed practices (classes) cannot be made up.

If a class has already started, please stop by the front desk before allowing your dancer to enter the classroom.

Students arriving 10 minutes or more late will not be permitted to participate in class that day.

Acknowledge by Initialing Below: *

Holiday Closures:
No make-up classes will be offered for any holiday closures—no exceptions.

We do not provide credits or refunds for missed classes due to holidays.

Gum is not permitted anywhere on studio premises.

Food and drinks must be consumed outside of the dance rooms. Water is allowed throughout the studio, provided it is in a sealed container.

Starbucks, Dutch Bros, or similar drinks are not allowed in the studio.  Students will be required to dispose of these items before entering the studio.

Please dispose of all trash and recycling in the appropriate bins. Keeping the studio clean is everyone’s responsibility.

Students who do not follow these guidelines may be dismissed from the studio.

Social Media & Photo Policy:
I understand that neither I nor my child may post any negative, inappropriate, or harmful comments regarding Elite Dance Academy (EDA), its parents, students, or staff, nor about other studios, teachers, or their students.

This policy also applies to all photos and videos. Any images or videos shared on social media must be respectful and appropriate. Posting content that is offensive, misleading, or damaging to EDA or others is strictly prohibited.

Families are not permitted to post photos or videos of other students without permission from that child’s parent or guardian. Elite Dance Academy is not responsible for the actions of families or students regarding social media use.

Backstage privacy is strictly enforced. No photos or videos are permitted backstage or in dressing areas at any time.

Anyone found in violation of this policy may be subject to immediate removal from the studio.

Parents are responsible for monitoring their child’s social media activity.

Photo and Video Release:

I grant Elite Dance Academy permission to photograph and record video of my child during classes, rehearsals, and events for use in promotional and marketing materials, including brochures, advertisements, social media, and the studio website.

I understand that all such images and recordings are the property of Elite Dance Academy and may be used, reproduced, and copyrighted by the studio. Participation in the program constitutes consent unless a written request to opt out is submitted.

Parent/Guardian Responsibility – Studio Communication & Awareness:
It is the responsibility of each parent/guardian to stay informed of all studio activities, including but not limited to Show & Shine weeks, recital rehearsals, picture days, performance dates and times, additional classes, and studio closures.

The studio communicates important information through bulletin boards, email, the Band App, and social media. Parents/guardians are expected to regularly check these platforms for updates.

It is also the responsibility of the parent/guardian to ensure that all contact information, including phone numbers and email addresses, is current and up to date with the studio.

Drop-Off, Pick-Up & Lobby Policy:
*

To maintain a safe, calm, and focused environment for all dancers, it is essential that all families strictly follow our lobby and drop-off procedures.

Lobby access is limited to one parent or guardian per dancer and is reserved only for families with dancers enrolled in Parent & Me (18–36 months) and Twinkle Tots (ages 2–5).

Dancers ages 5 and up must be dropped off and picked up at the front door. For safety, dancers under the age of 12 may not cross the parking lot without an adult. Parents/guardians must come to the door for pick-up.

Care of Students-Acknowledge by Initialing Below: *

Care of Students:
Elite Dance Academy does not provide childcare before or after class. Students should not be left at the studio for extended periods before or after their scheduled class time.

Students must be picked up promptly at the end of class. A 10-minute grace period is allowed. After that time, a late pick-up fee of $15 will be charged for every additional 5 minutes. Fees will be automatically charged to the card on file.

In the event of an emergency, please contact the studio as soon as possible.

For safety reasons, students under the age of 12 may not leave the building without being accompanied by an adult. Parents/guardians must come into the studio to pick up their child.

Studio Lobby Expectations
Acknowledge by Initialing Below:

Lobby Expectations:
The lobby must remain quiet at all times. Excessive noise, conversations, and distractions directly interfere with class instruction and prevent our front desk staff from assisting families efficiently.

All visitors are expected to keep voices low at all times. Children in the lobby must remain seated, quiet, and supervised. Running, yelling, or disruptive behavior will not be tolerated.

If noise levels become disruptive or these expectations are not followed, you will be asked to step outside. Repeated issues may result in loss of lobby privileges.

New students experiencing separation anxiety may request temporary lobby access for their first two weeks.

*

Medical & Emergency Release- Acknowledge by Initialing Below:
As the parent(s) and/or legal guardian(s) of the minor student(s), I hereby grant permission to Elite Dance Academy and its representatives to seek and authorize emergency medical treatment by a qualified and licensed physician or medical provider in the event of illness or injury.

This authorization applies in circumstances where, in the opinion of the attending medical professional, immediate treatment is necessary to prevent serious harm, disfigurement, physical impairment, or undue discomfort, and a reasonable effort has been made to contact me.

I understand that I am responsible for any and all costs associated with such medical care.

*
