2018 RUNNING CLUB
This form is STEP 1 in a two-step process. PAYMENT ON THE YMCA's SYSTEM MUST OCCUR BEFORE REGISTRATION IS COMPLETE. GO TO
OR STOP BY THE SUPERIOR YMCA TO PAY THE $30(member) or $60(non-member) FEE.
Youth Running Club will begin June 11 at 5:30PM. Check-in will begin at 5:00PM. Completion of this form AND payment must be completed before June 11. After June 11 all practices and races will be at various destinations throughout the Superior/Duluth area. Specific schedule will be emailed to all participants prior to June 11.

I hereby give my permission and approval for the above-named child to participate in all T-Ball activities during the 2018 season. I assume all risks and hazards incidental to such participation, and I release the YMCA, supervisors, and coaches from any claim arising from an incident or injury to my child. I also understand that the my child's photo might be used for future promotional efforts and if I object, I must communicate these wishes in an email or in writing to Jon Reimer the YMCA's Sports & Aquatic Director. By answering this question, this will serve as my digital signature.
