RESNA Assistive Technology Professional (ATP) Certification Scholarship Application
Funding is available to qualified employees of County Boards of Developmental Disabilities (CBDD) in Ohio who are interested in obtaining their ATP certification. I intend to complete the RESNA ATP Fundamentals course and submit proof of course payment and completion to my CBDD with appropriate turn-around time such that the CBDD can invoice OCALI by December 4, 2020.

I intend to take the RESNA certification test through the Prometric testing center, submit proof of certification registration fees (RESNA and Prometric) and provide proof that the ATP certification has been obtained to my CBDD with appropriate turn-around time such that the CBDD can invoice OCALI by December 4, 2020.

If accepted into this program, I understand that I am responsible for paying any fees for the ATP Fundamentals course and/or ATP certification in advance. I will be reimbursed by my CBDD, after the CBDD’s receipt of payment from the Educational Service Center of Central Ohio on behalf of the OCALI AT&AEM Center and following successful completion and evidence of activities as outlined. I have discussed this with my supervisor and CBDD finance officer and I have approval to proceed with this application process.
