Welcome to the Faith Formation Program at Sacred Heart Church! This form is to be used for those students who are both new to the Sacred Heart Faith Formation program and who are interested in registering for one of our programs for 2024-2025. Families of new student MUST be registered parishioners of Sacred Heart Church.

Fees for the 2024-2025 programs for grades 1-8 are as follows:
1 child - $125
2 children - $155
3 or more children - $185

Additional Fees (if applicable)
Bridges to Faith Curriculum Catch Up - $75
First Reconciliation/Eucharist - $75
Confirmation - $100
