International Service Trip Application

This application is required of anyone interested in attending an Inspiring Hands service trip to Dominican Republic in 2020. Individuals aged 18 or older can fill this out, minors ages 15 - 17 must complete a separate application and will need a parent in attendance if accepted on the service trip.

Applicants will need to provide the following documents upon acceptance into one of our trips: Volunteer Participant Agreement, Liability Waiver, Media Consent Form, $45 nonrefundable deposit and copies of passport, health insurance card (if applicable) and traveler's insurance (if applicable). These forms will be emailed to you once your application has been received and reviewed.
