Take A Break Form
This is my 2 week notice to drop all group classes or for all private lessons at APA.  You must drop before the 15th of the month in order to cancel by the 1st. (Ex: Drop by September 14th to stop October's payment).  Every parent has initialed our drop policy at time of registration.  

We cannot refund any tuition that you have already paid but we will put a credit in your APA account for any unused lessons for future use.
