Once you have filled out the form we will be in contact! Please note: we are an entirely volunteer run organization and we do our best to respond to forms as quickly as possible. Please be patient with us!

By filling out this form you certify that you have read and agree to the Pet Pantry's program policies:

1. I certify that the items I am donating are not expired, damaged, or in poor condition.
2. I understand that my donation to Lexington Pet Pantry is tax deductible to the extent allowed by law. 
3. I understand that I will not receive anything in exchange for the donation.
4. I understand that the Pantry cannot take expired food or treats, prescription medication, live feed (i.e., mice, crickets)
5. I understand that opened bags of food are NOT accepted at this time.
