Myriad is happy to reserve tables for large groups and special occasions! Please use this form to request a reservation. Once we receive your form, we will review it and will reach out regarding availability, cost, and other details. 

*Reservation fees vary based on your needs, size of your group, date you select, and how much time you need to setup.
