NTCA Record Book Attestation Form
The NTCA has compiled an All-Time Record book for NCAA Bowling. With large thanks to Mike Karushaar and past/present board members, we have assembled most stats from 2014 onward. In an effort to make the record book as accurate as possible, we ask you to please use this form if you believe you have a record that should be and is not currently represented. 

Record statistics that require the use of Bowl Sheet (ex: Strike %, Single Pin Spare %, etc.) are only being counted from the inception of the app from 2017-18 onward to ensure credibility and integrity. Paper submissions from personal tracking pre-2017 is not a valid attestation. High baker match, traditional match, and best-of-seven baker scores will be accepted pre-2017 granted a proper documentation of proof is presented. Proper documentation should include a scan of the used score reporting form from the day of the record in question and/or an article from the athletic departments website citing the score in question. 

Please submit one form per program. If you have multiple records to attest, please note all of those in this form. For documentation, please e-mail the NTCA (ntcabowling@gmail.com) with the subject line "Record Book - (School Name)." For example, Subject Line: "Record Book - ABC University".
