Public Feedback Form: UW's 2016 Biannual Emergency Management Plan Update
Thank you for taking time to provide your feedback for the biannual update of the 2016 UW All-Hazards Comprehensive Emergency Management Plan (CEMP) , Your comments may be submitted anonymously. However, if we have questions about your online submissions or need further clarification, only those surveys containing valid contact information may receive follow-up assistance.

There is no limit to the number of comments each person may submit. However, each online survey limits respondents to 10 separate comments, questions or suggestions per entry. Additional comments over 10 may be submitted via a web link provided when you complete the initial survey.
