This form is to be submitted when a player is suspended or ordered off the court in ANY NNL fixture.
Please give as much detail about the game, player and reason for the suspension or ordering off as possible.
BOTH  Umpires should submit an independent report within 48hrs of the match for consideration by the NNL Committee.
