Please complete this form for each individual applying to join the Alumni Delegation (i.e. couples or those hoping to participate with a friend/relative must submit separate applications--one for each person). Once you have completed the application, you should receive a record of your responses at the primary email address you provide below.
In addition to completing the online application, applicants will also be asked to submit a $150 application deposit. The application deposit will be applied to the balance owed for participation in the program and is refundable through May 15, 2017. You will receive additional information about this fee and how to submit it after you have submitted a completed application.
