Please complete this form to register your child for our Richard Wright / OST After-School Program. Please note you must complete a form for EACH CHILD! THIS IS NOT FOR MULTIPLE CHILDREN!
Please Note: We will be opening up registration to the first 60 students (including 10 Kindergarteners). Registration is on a first come, first serve basis. Any student after the 60 enrolled will be placed on a waiting list. Thank you for your understanding.
Richard Wright’s OST After-School Program will begin on Monday, September 11th and run weekly on MONDAYS through FRIDAYS from dismissal to 5:30 PM. We strongly encourage parents and guardians to pick up students starting at 5:00 PM. If you opt to have your children walk home, a waiver will be available for you to sign allowing your child(ren) to do so. Please note, we will require ID for those who are cleared to pick up students to ensure your child's safety!
~ NOTE: This form has multiple sections and will take about 10 minutes to complete. Please answer all questions completely. Failure to do so will delay your child's enrollments into the program.
~HEALTH FORMS must be completed by your child's physical health form which is located on the last page of this form. You can email the health form for your child to joni@legacyyte.org prior to them starting in the program.
~ Parent email address and a current cell phone number is very important.
This program is for students who currently attend Richard Wright Elementary School on one submission form. If you need to register more students, you will need to complete the form multiple times. Please be sure to answer each question completely.
