HSLC Animal Check Out Request Form

Each individual classroom teacher requesting an animal must complete the form below for themselves. 

Make sure you are logged into your PISD google account to view all documents 

Please thoroughly read the care guidelines (https://tinyurl.com/HSLC-Pet-Guide) and care sheet for your preferred species of animal (Care Sheets) before submitting a request.

Please allow 2 school days between your request and your intended pick up date.

Please message us if you have not received confirmation of your request within 2 days of submission. Additional time to prepare requested critters may be needed between April 1st and the end of the school year due to increased request volume during this time.

Please select each animal you would like.  HSLC staff will determine and provide the appropriate equipment needed for each animal at checkout.

Please keep in mind that birds are very vocal and can be disruptive in some classroom situations.

Fish/Aquarium Set-Ups
Aquariums should be kept set up on your campus for the entire school year.

Aquariums must be set up and "cycled" (allows the water to build up beneficial bacteria and stabilize) for at least two weeks prior to animal pick up. Aquariums are unable to come back to the Center for holiday boarding because of this.

If the aquarium remains set up and functioning in your classroom, the fish/axolotl may be transported in a temporary, aquatic carrier back to the Center for select breaks for boarding.

Please remember that it is recommended to keep 10 gallons of water for 1 inch of aquatic animal.
