2019 Nonprofit and Community Excellence Awards Nomination Form
This award identifies outstanding companies and leaders who have supported nonprofit organizations, along with nonprofits and nonprofit executives who have made a significant impact on the community over the last year.

Corporate Citizen of the Year
Awarded to a for-profit organization in recognition of its impact to a single nonprofit organization or multiple organizations, through financial contributions and collective efforts to promote, encourage and organize fundraising efforts and company sponsored programs. The award also recognizes a company that engages its employees to participate in volunteerism.
Awards will be given in the small employer category (less than 100 employees) and the large employer category, (100 or more employees).

Next Generation Leadership
This award is for a for-profit executive under the age of 40 who demonstrates leadership and significant contributions to a nonprofit organization(s), through active leadership, board work, and/or strategic management and guidance.
Corporate Volunteer of the Year

Corporate Volunteer of the Year
Given to a for-profit, non-executive volunteer demonstrating outstanding dedication to a nonprofit, or multiple organizations. Can be ongoing, or linked to a specific single project or cause.

Partnership Award
Given to a for-profit organization that can best articulate a successful, measurable business outcome from their community service efforts, exemplifying the principle that doing good is also good business.

Nonprofit Organization of the Year
This award is given to a nonprofit in recognition for creativity and innovation in building a sustainable organization as well as excellence in teamwork and an outstanding dedication to mission in the community, with emphasis on excellence in management and operations.

Nonprofit Collaboration of the Year
Recognizing a collaborative project that engages relationships with multiple nonprofit organizations and multiple shareholders within the community and that helps solve a community problem or issue. The winning organization will have shown innovation in approaching a community problem or issue leveraging relationships to make meaningful impact in the community. Note: since this award recognizes a project that may involve multiple organizations or individuals, it may be awarded to more than one party, since it is the collaboration that will be recognized.

Nonprofit Executive of the Year
This award is given to a nonprofit leader who demonstrates vision and innovation in advancing the mission of his/her organization.The award will recognize a leader who has made a substantial difference (financial or otherwise) in their organization over the last 12 months.

Benefit Corporation of the Year
This award recognizes a registered benefit corporation in the state of Connecticut that has demonstrated creative application of the principles of social enterprise in the operation and funding of the organization.

•Each Nonprofit and Community Excellence Awards winner will be selected from the field of nominees in each category listed above by a panel of independent judges made up of selected professionals and community leaders.

•The Nonprofit and Community Excellence Awards recipients must be individuals or organizations operating or based in the 61 towns of central Connecticut: Andover, Ashford, Avon, Barkhamsted, Berlin, Bloomfield, Bolton, Bristol, Burlington, Canton, Chaplin, Cheshire, Colchester, Columbia, Coventry, Cromwell, Durham, East Granby, East Hampton, East Hartford, East Windsor, Ellington, Enfield, Farmington, Glastonbury, Granby, Haddam, Hartford, Hartland, Harwinton, Hebron, Lebanon, Manchester, Mansfield, Marlborough, Meriden, Middlefield, Middletown, New Britain, New Hartford, Newington, Plainville, Plymouth, Portland, Rocky Hill, Simsbury, Somers, South Windsor, Southington, Stafford, Suffield, Tolland, Union, Vernon, Wallingford, West Hartford, Wethersfield, Willington, Winchester, Windham, Windsor, and Windsor Locks
