Lacrosse Camp Liability Waiver and Release Form
Acknowledgment and Acceptance of Risks: By participating in the lacrosse camp operated by Patrick and Sean Crogan, the participant (or the participant’s parent/guardian if under the age of 18) acknowledges and accepts the risks associated with lacrosse, which include but are not limited to bodily injury, falls, contact with other participants, equipment, or other unforeseen risks. Participation in the camp is voluntary and all activities are undertaken at the participant’s own risk.
Waiver and Release of Liability: In consideration of being allowed to participate in Crogan Brothers Lacrosse Camp, I, the undersigned, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Crogan Brothers Lacrosse, its officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasees”), from any and all injuries, disabilities, death, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.
Health and Safety: I certify that the participant is in good health and physically capable of participating in all activities involved in the camp. In the event of an emergency, I give permission for camp staff to seek medical treatment for the participant if they are unable to reach a parent/guardian.
Media Consent: I authorize Crogan Brothers Lacrosse to use photos and videos of my child taken during camp activities for promotional and business purposes. This may include use on Instagram, websites, flyers, and other media.
Refund Policy: Because Crogan Brothers Lacrosse operates this camp on a budget and makes commitments based on enrollment, registration fees are non refundable except in special circumstances. Any refund request must be submitted in writing and will be considered on a case by case basis at the sole discretion of Crogan Brothers Lacrosse. Approval of any refund, including whether it is full or partial, will depend on the specific circumstances.
Acknowledgment of Understanding: I have read this release of liability and assumption of risk agreement, and I fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature for this to be a complete and unconditional release of all liability to the greatest extent allowed by law.
