Policy 915 Fundraiser Request(s) Form
This form is to be used to submit a request for fundraising activities to the building principal or Director of Athletics.  Requests should be sent no later than forty-five (45) days prior to the event, and must be reviewed and approved at the building/athletic office level before any such fundraising may occur.  A response to a request(s) will be received within five (5) business days after submission.  If a response is not received within five (5) business days after submission or details of the fundraiser(s) change, please contact the appropriate building administrator(s) or Director of Athletics.
