Please complete this form if you have a child that will be 5 years of age on or before September 1, 2025 OR if you are applying for Early Entrance Kindergarten (EEK) because your child will be 5 years on or after September 2, 2024 but on or before October 15, 2024.  You can find EEK information HERE
