*This form is to request a spot in our School Holiday Mini Camp program. Each day or week will be a new camp/new enrollment.

*Once you fill out the form you will receive an email informing you that your request has been received. Confirmation of the enrollment will come when the child has been enrolled and an email with go out informing you of the enrollment with a link to pay for the services.

*Enrollments are week to week (or each day for 1 days holidays). Payment for the week/day must be made the Thursday before. Any changes to the enrollment must be made 5 days in advance through our online Changes Form. If the Change is not made in this time frame then you WILL be responsible for the full tuition. Attendance does not reflect tuition the charges are based on enrollment.

*If payment has not been collected for the previous camp future enrollments will not be permitted until the past due balance is paid in FULL

*Early and extended care is available for an additional $5.00 per day (whether you need both or just one option it is a $5.00 additional charge per day)

* During Christmas and Spring Break ONLY Field Trips are on Fridays for an additional fee, price varies based on the destination. Children 6 and up are eligible to attended field trips, All field trips are chaperoned by staff only. Please see our Summer Flyer for detailed information on field trips.

*If there are any notes that you would like to provide please do so by selecting "Other" followed by any information you need to provide.

*Please send a sack lunch each day unless notified by us that lunch will be provided.

PAYMENT POLICY: Payments for Summer Camp are charged weekly, these charges must be paid in FULL the Thursday prior to the week of Camp. Charges are NOT adjusted for days a camper is absent. If the account is unpaid for the previous week future enrollments are not permitted until the past due balance is paid in FULL.
ATTENDANCE POLICY: Any changes to the enrollment for a week of camp must be made by the Thursday before that week in order for the change to be reflected on your tuition. *Reminder that charges are not adjusted due to being absent.
