South Columbia Baptist Church Youth Event Permission Form
We're having lunch, mini golf and ice cream! We will eat together right after church and leave for mini golf around 12:45 PM. We will return back to church for dessert. Pick up time from church is at 3:30 PM. Students may bring friends. Please let Jim know. Completed permission slip is required for each person attending.

I grant permission for my child(ren) to have lunch and participate in mini golf at Columbia Sports Park with the Youth Ministry on 5/27/2018. I recognize that every attempt will be made by the ministry leadership to provide a safe event, transportation to/from this event, and reasonable numbers of chaperones. This event includes potentially dangerous activities such as riding in a motor vehicle. Ministry activities will be supervised. Participants must adhere to safety instructions given at time of event by leadership.
