    Student Information Registration

Polk County Students who need to take or retake a Florida End-of-Course (EOC) Assessment may participate in the Summer 2017 Session during the Florida Department of Education’s (FLDOE) window of July 10 – 13, 2017. Our district will administer the following assessments:  FSA Algebra 1 EOC (For students who completed Algebra 1 for the first time in the 2014-15 or subsequent school years)  Algebra 1 EOC Retake (For students who completed Algebra 1 for the first time PRIOR to the 2014-2015 school year)  FSA Geometry EOC  FSA Algebra 2 EOC  Biology EOC  Civics EOC  US History EOC In order to take advantage of this opportunity, this Online Registration Form must be completed by June 16, 2017. Submission of your registration form will reserve a spot for your student and provide us with the necessary information to enter the student into the state testing platforms. The Testing Coordinator at your testing site will contact you via email or phone on or before July 7th to verify registration and provide you with additional information, such as testing procedures and specific testing times. No late registration is available for these assessments. The FSA EOC exams involve two testing sessions over two days. These sessions are 90 minutes each, but the student may have up to 3 ½ hours each day. Please note that typically the Florida Department of Education allows students to take both sessions in one day for the summer administration only. For the science and history EOC exams, as well as the NGSSS Algebra 1 EOC Retake, students are required to sit for at least 80 minutes of each assessment and will not be permitted to leave (even if completed and submitted) prior to this time limit. For most students, each of these EOCs takes approximately 3 hours to complete; however, students may continue testing until the end of the school day in order to complete the assessment. Snacks or lunch will not be provided. However, a 30-minute break is provided for a student to eat after the state-required 160-minute sitting for the non-FSA assessments.
